Jordan Hansen, MA, LADC

Chief Executive Officer / Co-Founder

Jordan is an experienced leader focused on delivering value to healthcare systems through implementation of effective, integrated care for people with substance use disorders. Jordan previously was the Vice President of Strategy and Growth for a recovery technology company, and before that, was the Clinical Director of Hazelden Betty Ford’s Professional Education Solutions team. In that role, he built a team that provided innovative solutions at the federal, state, local and program level.

Previous areas of focus include developing and executing on strategies for technology and content development, B2B and B2C product and business development, and various consulting and training strategies around establishing or improving integrated recovery programs.

He is particularly interested in building innovative technology-driven services to address inequity and system dysfunction, helping to establish effective, low-threshold medication-assisted treatment programs in emergency departments, correctional settings, primary care clinics, and specialty care facilities; assisting with strategic development of effective, recovery-oriented systems of care in rural areas and among underserved populations; and affecting cultural shifts across service systems.

Philip Gyura, DNP

Head of Medical Innovation / Co-Founder

Dr. Gyura is a leader in the development and application of mobile health solutions for substance use, HIV, and Hepatitis C prevention and treatment. He also has worked on implementing specialty practices within primary care in high-acuity areas using patient-centered care models and helped bring quality care to marginalized populations. He is a graduate of the University of Minnesota and Columbia University

Dr. Gyura is passionate about problem solving within our systems of care and how to best use current resources to bring unique models of care to those in need. His drive for teaching lead him to precept new nurse practitioners, facilitate novice to experienced providers in getting DATA waivers, and train providers in the use of PEP/PREP in primary care.

Dr. Gyura’s current interests include bringing care to people using substances in all stages of recovery, and creating safe places where all are welcome along their journey.

Dr. Steve Delisi, MD, FASAM, DABAM

President, YourPath Care

Dr. Delisi is board certified by both the American Board of Psychiatry and Neurology and the American Board of Addiction Medicine. Before joining the YourPath team as CMO and President, Dr, Delisi had served as a psychiatrist for the Hazelden Betty Ford Foundation from 2005-2023.

During his time with Hazelden, Dr. Delisi most recently served as Medical Director, Enterprise Solutions. Prior to that role, he also served as Medical Director of Professional Education, Assistant Dean of the Hazelden Betty Ford Graduate School of Addiction Studies, and Midwest Regional Medical Director for Recovery Services at Hazelden Betty Ford. Dr. Delisi continues as an Adjunct Faculty member of the Hazelden Betty Ford Graduate School.

Prior to joining Hazelden Betty Ford, he held positions as Director of Psychiatric Services at Rush Behavioral Health-DuPage and Associate Director of the Psychiatric Residency Program at Rush University Medical Center. Dr. Delisi is a nationally recognized leader, speaker, and consultant on the use of evidence-based, integrated treatments for co-occurring and substance use disorders, as well as the risk conferred by the epigenetic effects of Adverse Childhood Experiences and trauma.

Jeffrey Larson, MA, LADC

Head of Recovery Innovation / Co-Founder

Jeff has spent his life working to improve the lives of people who struggle with substance use disorder and its devastating impact. He has served as a counselor in residential treatment, outpatient programs and halfway house settings. He has been a treatment program director, designed programs and led organizations that provide treatment services as well as complimentary services that support people in recovery.

During his career, Jeff served as Executive Director of a rural non-profit organization focused on increasing access to treatment services, housing, and recovery support services. He was Executive Director of Health and Human Services for the Mille Lacs Band of Ojibwe, a tribal nation in Minnesota where he had oversight of treatment services, medical clinics, and a variety of public health and community support services. Most recently, he was involved with Livio Health Group, a start up in Minnesota focused on mobile SUD care and the integration of peer support services focused on increasing access to care for Medicaid recipients.

In all of his roles, his primary focus has been on improving access to care to people who have been marginalized through poverty, racial injustice, and stigma. He is a strong advocate for person-centered care that meets people where they are, and is focused on providing effective, longitudinal, recovery supports.
