We started YourPath with a simple idea:

What if care for substance use disorders was easy to access, truly person-centered, and embedded within a network of care?

A simple idea, but one that is already transforming the way substance use disorders are treated, providing immediate, effective and culturally responsive care to thousands of patients across the state of Minnesota.

We all know how the current system works. It’s siloed, expensive and inefficient, often driven by ideologies that put the patient in the back seat of their recovery journey. Not to mention that it’s easier to access drugs than it is to access recovery. We wanted to flip that around.  

What if instead of keeping patients in siloed, expensive and ineffective systems of care, we helped them find the care that works for them, providing supportive services across their recovery journey and filling the gaps in the current, fragmented system?

What if instead of telling patients what their recovery should look like, we let the patients tell us what their recovery should look like?

What if a patient could simply send a text message, anytime of the day, and be immediately connected to a peer recovery specialist who could provide support and help them access services?

In a little under two years, we have begun to realize our vision. Our team, consisting of nurse practitioners, masters-level counselors, peer recovery specialists and healthcare technology experts, have helped thousands of patients. Assisted by our technology platform, we have stayed engaged with these patients, providing medical and clinical care as needed, and helping them find more permanent homes for their recovery and healthcare needs.

It turns out that when you put the patient in the driver’s seat of their recovery, amazing things happen.
